New Patients

Garland Eye Associates now offers our current patients and new patients access to our Patient Easy Access Portal. Our patient portal is a window through which patients are able to communicate with our office. Garland Eye Associates’ patient portal will enable patients to log into a secure website and send a secure email, complete new patient information and update previous patient information.

The purpose of a patient portal is to provide patients with secure and easy access to their up-to-date medical information. To register for the patient portal please select the Patient Login button at the top of our site.

The following three forms below have been provided for your convenience. You will need Acrobat Reader to view the pages. If you do not have Acrobat Reader, you can obtain a free download. Once the pages are open you can print them out. Please fill out the pages as fully as possible and bring them with you to your scheduled appointment.
